Took the head off again so I could try getting the cylinder off. Clymer's says it should be secured to the crankcase with screws, but in the case of the CM91 it appears to simply be held in by the head itself.
It came off pretty easily, with just a little tugging; no mallet or screwdriver prying required. I pulled it off far enough to get a look at the piston but by that point my garage session was coming to a close, so I just reassembled.
Aside from the head itself, only the cam chain roller has to be removed in order to get the cylinder off. Don't lose the copper washer sandwiched between the roller and cylinder wall!
